    (1) Is the Pet Lost or Found? 
    (2) Is it a Cat, Dog, Rabbit, etc;
    (3) Physical  Description  (age, gender, color, collars, tags); 
    (4) Where was the Animal Lost/Found? (Street Name, City);
    (5)  Date Lost or Found; 
    (6) How can we contact you? - email, phone numbers.
